Lowering uric acid levels

The treatment of gout aims to relieve pain and inflammation during acute attacks, prevent future attacks and lower uric acid levels in the blood to prevent the formation of urate crystals in the joints.

Standard urate-lowering medicines are:

  • Allopurinol

This is an oral prescription that helps lower serum uric acid levels.

  • Febuxostat

This is another urate-lowering therapy. This medication is taken orally and decreases the body’s production of uric acid. It can be taken by people with mild to moderate kidney or liver disease.

Colchicine add-on to allopurinol drastically reduces flare recurrence, when initiating a gout prevention therapy

Borstad GC et al. demonstrated that colchicine
0.6 mg reduced the frequency of gout flares11

When initiating therapy to lower urate levels, 75% of the patients will experience one to several new  gout flares within first 6 months13.

When preventing recurrent gout flares colchicine in low dose can significantly reduce the number of flares when given in combination with uricosuric drugs12.

42 patients starting allopurinol were randomized to receive colchicine 0.6 mg or placebo for up to 6 months. The chart shows mean number of acute gout flares at the 0-3 and 3-6 month time periods, and overall (n=43; colchicine=21, placebo=22)

GOUT treatment

Reduction of inflammation

The common symptoms of gout flares are inflammation and pain.

Pain management

There are several options given to provide gout pain relief. It is important to prescribe medicines which eliminate the inflammation as well.

Role of colchicine for acute gout flare & prevention gout treatment.

Optimal colchicine dosages

To reduce number of adverse events American College of Rheumatology 2020 Guideline for the Management of Gout recommend to use colchicine in low dosages.
